1-5 employees* ($210)+ Admin Fee
6-10 employees* ($300)+ Admin Fee
11-25 employees* ($400)+ Admin Fee
26-50 employees* ($525)+ Admin Fee
50-100 employees* ($775)+ Admin Fee
Over 100 employees* ($975)+ Admin Fee

**The definition of employee includes anyone providing services to member company on a full or part-time basis, receives compensation from member company for those services, and does so regularly through-out the year. Contractors and part-time employees equate 2:1.

A one-time $50 admin fee should be included with payment.
